Person Centred Care ProgramsThere is much talk about person centred care but few really understand the implications and detail to assessment, documentation and role of the registered nurse to genuinely achieve person centred care for each resident or patient. 8 CPD hours This is a step by step program assisting registered nurses to introduce genuine person centred care plans, that stand up to scrutiny. You identify one (1) resident for assessment and care plan development. This may be a resident with 'behaviours' and or with a dementia syndrome. We demonstrate how to:
Suitable for small facilities or units of approx. 30 beds, including dementia specific units, our report will assist you to sustain the change. It also includes CPD hours for nurses and much of the Easy Start program. For large organisations introduce the program to one unit, then you gradually ‘role it out’ to all units or sites, using your now experienced staff and our report as a guide. With this format, we are with staff as they take the first step of person centred care, of moving away from the morning routine of waking residents for breakfast/showers. We are ‘on the floor’ with staff, across the shifts, as they experience the change and flow-one effect.
This program supports your Person Centred Care philosophy and cements practices and includes a minimum of 12 CPD hours for nurses and most of the Easy Start and Kick Start programs.
